Thoracic Surgical Oncology

Thoracic Surgical Oncology is a subspecialty of thoracic surgery that focuses on the surgical management of cancers affecting the thoracic organs, particularly the lungs, oesophagus, and mediastinum. Thoracic surgical oncologists work to remove tumours, manage cancer-related complications, and improve patient outcomes through targeted surgical interventions.

Types of Procedures in Thoracic Surgical Oncology

Lobectomy:
  • Procedure: Removal of one lobe of the lung. This is often performed for early-stage lung cancer.
  • Approach: Can be done via traditional thoracotomy or minimally invasive techniques like video-assisted thoracic surgery (VATS) or robotic-assisted surgery.
  Pneumonectomy:
  • Procedure: Complete removal of one lung. This is usually reserved for cases where the cancer is large or centrally located.
  • Approach: Typically performed through a thoracotomy, though minimally invasive approaches may be used in some cases.
  Segmentectomy:
  • Procedure: Removal of a segment of a lobe of the lung. This is often used for small, early-stage tumours or when preserving lung function is crucial.
  • Approach: Usually performed via VATS or robotic-assisted techniques.
  Wedge Resection:
  • Procedure: Removal of a small, wedge-shaped portion of the lung containing the tumour. This is typically used for small tumours or as a diagnostic procedure.
  • Approach: Can be done via VATS or robotic surgery.
  Esophagectomy:
  • Procedure: Removal of part or all of the oesophagus, often to treat oesophageal cancer. This may include reconstruction of the oesophagus using parts of the stomach or intestines.
  • Approach: Can be performed through open surgery, VATS, or robotic-assisted techniques.
  Mediastinoscopy:
  • Procedure: Involves using a mediastinoscope to obtain tissue samples from lymph nodes and other structures in the mediastinum. It is used for staging lung cancer and assessing lymph node involvement.
  • Approach: Performed through a small incision above the sternum.
  Endobronchial Ultrasound (EBUS):
  • Procedure: Combines bronchoscopy with ultrasound to visualise and biopsy lymph nodes and other structures within the chest. It is useful for staging lung cancer and guiding treatment decisions.
  • Approach: Performed through the bronchoscope inserted into the airways.
  Pleurectomy/Decortication:
  • Procedure: Removal of the pleura (lining of the lungs) and, if needed, fibrous tissue that has formed around the lung. This is often done for malignant pleural mesothelioma or chronic pleural effusion.
  • Approach: Typically performed through a thoracotomy or VATS.
  Chest Wall Resection and Reconstruction:
  • Procedure: Removal of part or all of the chest wall due to tumours, followed by reconstruction using synthetic materials or tissue grafts.
  • Approach: Often involves a thoracotomy, with reconstruction performed as needed.
  Thoracic Aneurysm Repair:
  • Procedure: Repair of an aneurysm in the aorta or other major thoracic arteries, often performed if cancer or other conditions have affected the vascular structures.
  • Approach: May be done through open surgery or endovascular techniques.
  Robotic-Assisted Thoracic Surgery:
  • Procedure: Uses robotic systems to perform precise and minimally invasive surgical interventions for tumours in the thoracic cavity.
  • Approach: Allows for enhanced dexterity and visualisation compared to traditional methods.
  Cryotherapy:
  • Procedure: Uses extreme cold to destroy cancer cells, often applied to tumours in the lungs or other thoracic areas.
  • Approach: Typically done via a bronchoscope or through a needle inserted into the tumour.
  Radiotherapy:
  • Procedure: Uses targeted radiation to treat cancer in the thoracic region. While not a surgical procedure, it is often used in conjunction with surgery for managing thoracic malignancies.
  • Approach: Delivered externally or internally (brachytherapy).
